Supportive Housing Services

Our Supportive Housing Services are grounded in the belief that permanent, stable housing is the foundation for long-term success. We work to promote housing stability for individuals experiencing homelessness, particularly those facing increased barriers to housing, including but not limited to: domestic violence, extremely low income, previous criminal history, prior foster care involvement, separated families, serious mental illness, and substance use disorders.

Our services focus on support, coordination, and connection to resources that help individuals remain housed and improve overall well-being.

Services We Provide to Support Housing Stability:

Resident Service Coordination

We support low-income residents in maintaining housing stability and independent living. Through service coordination and referrals, we help residents stay connected to community resources, reduce isolation, and improve health and safety. These supportive services help residents remain housed while also reducing preventable healthcare and crisis-related costs.

We do not provide financial assistance or direct payments for housing or healthcare.

Housing Navigation

We assist at-risk and low-income individuals in identifying and accessing affordable housing options. Housing Navigation services include help locating housing resources and available properties, understanding application requirements, and connecting with community-based housing providers. Housing Navigation services are supportive and informational only. We do not provide rental assistance, deposits, or direct housing placement.

H2O Pre-Tenancy

The H2O Program serves individuals who are experiencing or at risk of homelessness, including individuals with Serious Mental Illness (SMI), and who are AHCCCS members. The program provides pre-tenancy and tenancy-sustaining services designed to support long-term housing stability. Services are delivered through skill-building to enhance tenancy success, regular home visits and safety checks, and crisis-prevention strategies to help participants maintain stable housing.

New Mexico Supportive Housing Initiative

The NM Supportive Housing Initiative provides housing units and support services for individuals and families who are experiencing homelessness or precariously housed. As part of the New Mexico Behavioral Health Purchasing Collaborative Supportive Housing Plan, this program provides subsidized housing units for households with income limits ranging from 30-60% of the Area Median Income (AMI) who have a long term mental or physical condition that requires long-term supportive services.
